What is RDW (red blood cell distribution width)?

Red blood cell distribution width is a measure of how much your red blood cells differ from one another in size. Healthy red cells are fairly uniform, roughly 6 to 8 micrometers across, because your bone marrow produces them under tight control. When production is disrupted, cells of different sizes enter circulation, and the RDW rises.

The number comes free with any routine blood panel. An automated analyzer measures thousands of your red cells, plots their volumes on a curve, and reports how wide that curve is. According to the NIH’s clinical reference on red cell indices, RDW “represents the coefficient of variation of the red blood cell volume distribution and is expressed as a percentage,” with a normal value around 13 ± 1.5%. The wider the spread, the higher the RDW.

What is a normal RDW level?

RDW-SD is a direct measurement of the width of the red cell size curve. RDW-CV is that width expressed relative to the average cell size, which is why very small or very large average cells can shift it. Many labs report both. If your RDW-CV is borderline but your RDW-SD is clearly elevated, the RDW-SD is often the more reliable flag.

What does a high RDW mean?

A high RDW means your red blood cells are more variable in size than a healthy sample would be. It tells you that red cell production is disturbed, not why. That’s the key mental model: RDW is a smoke detector, not a diagnosis.

Because so many processes converge on red cell size, an elevated RDW shows up across a wide range of conditions.
