A clinical way to understand your symptoms

The Parsley Symptom Index, or PSI, is a proprietary health assessment developed by Parsley Health clinicians to evaluate how symptoms show up across the body.

Rather than looking at symptoms in isolation, the PSI helps identify patterns across multiple systems, offering a clearer picture of what may be driving how you feel.

It is used in Parsley care to guide clinical conversations and track progress over time, and it is also available here as a free starting point for anyone seeking clarity.

What you'll get from PSI in 5 minutes

Your results are educational, not diagnostic. They are designed to help you better understand your health and decide what to do next.

A personalized symptom score across nine body domains

Insight into which systems may need support

Context for how symptoms may be connected

Clear next-step options, whether you want to explore care, labs, or learn more

Why your symptoms matter

Together, your symptoms tell a story. The PSI helps make that story visible.

Symptoms are early signals

Symptoms often appear long before lab values cross a clinical threshold. Fatigue, brain fog, bloating, mood changes, or sleep issues can signal system imbalance years before a diagnosis is made.

Symptoms are interconnected

A single symptom rarely belongs to just one system. Stress affects digestion. Hormones affect mood. Blood sugar affects energy. Looking at symptoms together reveals connections that isolated tests can miss.

Your experience is data

Medicine tends to prioritize lab values over patient experience. The PSI treats how you feel as essential clinical information—capturing changes in quality of life, not just numbers on a page.

The 9 body domains assessed

The PSI evaluates symptoms across nine interconnected body domains that influence daily health and long-term wellbeing:

01

Neurological: Mood, focus, memory, and cognitive clarity

02

Musculoskeletal: Pain, inflammation, mobility, and recovery

03

Gastrointestinal: Digestion, absorption, immunity, and gut balance

04

Metabolic: Blood sugar regulation, energy, and weight patterns

05

Respiratory: Breathing, immune load, and environmental sensitivity

06

Cardiac: Cardiovascular stamina, circulation, and resilience

07

Hair & Skin: Visible signals linked to hormones, nutrients, and inflammation

08

Mental & Emotional: Stress response, sleep, and emotional regulation

09

Reproductive & Genital: Hormonal patterns, menstrual health, libido, and fertility

Built by clinicians, grounded in care

The Parsley Symptom Index was developed by Dr. Robin Berzin, MD, founder, and CEO of Parsley Health, in collaboration with Parsley’s senior clinical team.

The PSI is a patient-reported outcomes survey created to help clinicians and patients track how health changes over time. It is designed in alignment with FDA guidance and has been clinically-validated. It reflects decades of clinical experience caring for people with complex, persistent symptoms and is continuously refined as Parsley’s care evolves.

How the PSI fits into care

Parsley members complete the PSI before their first visit and periodically over time. Clinicians use it to guide conversations, monitor progress, and understand how changes in care affect quality of life. Nearly 9 out of 10 members report symptom improvement within their first year of care.
